Nurse LPN Evening Supervisor

Responsible for the day-to-day supervision of Brookdale’s resident care staff within assigned community(ies).

  1. Provides direct supervision of at least two or more full-time time community care staff, which includes monitoring job performance, conducting associate performance evaluations, coaching and counseling associates and conducting and documenting corrective action as needed. Participates in the hiring, firing, and disciplinary processes.
  2. Makes daily rounds to ensure all resident care staff is performing work assignments in accordance with acceptable care standards, laws and regulations. Reviews resident care notes for completeness; makes sure they are descriptive of the care being provided and reflect the resident’s response to the care. Reviews change of shift logs to ensure actual or potential change of residents’ conditions are captured.
  3. Communicates to families regarding communities' personal needs and achievements. Reminds families of need for scheduled physician, dental, or eye appointments, as indicated on PSP.
  4. Performs daily and monthly safety and infection control checks of the home and grounds, assisting the Community Director for community health and safety purposes.
  5. Coordinates training and education of community care staff on an ongoing basis.
  6. Verifies all associate hours on time cards and weekly hour’s report. Completes resident care associate schedules to assure proper coverage to meet community needs and according to budget. Coordinates monthly staff schedules, arranges replacement staffing when necessary and maintains clear records of all changes, requests and irregularities. Develops resident care associate assignment tasks; develops shower and laundry schedules for resident care associates.
  7. Participates in the maintenance of community records including review of documentation performed by Community Assistants, monthly MAR reviews, setting up new community and thinning records.
  8. Participates in department quality improvement activities, to include completion of quality assurance surveys as assigned by Community Director. Participates in surveys and inspections made by authorized government agencies. Participates in the development of Plan of Corrections (POCs) in response to identified deficiencies.
  9. Trains resident care staff or coordinates training programs as required.
  10. Responds promptly to all emergency calls from communities, including communities residing in the independent section of the community. Shares on call duties as required.
  11. Participates in community functions including family socials and open houses.
  12. Develops and maintains a positive image with community. Represents the facility and the company to governmental agencies, professional organizations, community groups and other appropriate public agencies/groups.

Education and Experience
Associate's degree (A.A.) or equivalent from two-year college or technical school or higher level of educational. A minimum of two years’ related experience. Previous supervisory/ management experience preferred.


Certifications, Licenses, and Other Special Requirements
Registered Nursing License, if required by state. In accordance with state law, must possess current state certification and follow regulations to maintain certification currency. Basic CPR/First Aid training. Fire Safety training. Ability to work every other weekend or share weekend coverage with HWD.
