Resident Care Coord - LPN/LVN

Oversees the day-to-day operations of the community support services team and directly supervises health care provided within the community.
- Manages the health care of all residents, including the dissemination of information to families and staff.
- Performs on-site assessments of residents admitted to hospitals or skilled nursing facilities, and maintains contact with residents’ families with the intent of returning resident to Brookdale community.
- Completes a pre-admission assessment for each resident and updates every six months or as needed. (Refer to state specific regulations regarding frequency of pre-admission assessments).
- Initiates a resident service plan according to the individualized needs of the resident, as prescribed by physician and/or residence policy. Provides resident and family instruction. Where applicable, seeks input from the resident and family in the development of the service plan.
- Performs ongoing assessments of residents' physical and psycho-social needs and coordinates with other departments to ensure quality, proactive care.
- Conducts pre-admission screening of prospective new residents. Ensures all required documentation is completed prior to or upon resident admission, including nursing assessments, service plans, and other assigned forms.
- Observes residents; records significant conditions and reactions, and notifies residence director and physician of resident's condition and reaction to drugs, treatments, and significant incidents.
- As appropriate, explains procedures and treatments to residents to gain cooperation, understanding, and allay apprehension.
- Communicates with resident's physicians regarding residents' status and condition changes.
- Facilitates continuity of care for those residents receiving home health care services.
- Provides training and education to resident care staff on an ongoing basis including classroom in-services and situation-specific training.
- Organizes or assists the community or district director in organizing, developing, and conducting service plan reviews, as required, with appropriate resident care team members and resident families.
- Assigns weekly blood pressure and weights to rotating shifts.
- Ensures in-house ancillary medical services, such as podiatrist, doctor visits, dental visits, psych visits, lab, X-ray, ambulance, etc. are scheduled and followed through.
- Ensures family members are aware of resident's need for scheduled medical, dental, or eye appointments.
- Responds to life saving situations based on nursing standards, policies, procedures, and protocol.

Education and Experience
Associate’s Degree in Nursing and a minimum of two to four years relevant experience.
Certifications, Licenses, and Other Special Requirements
Current State LPN or LVN License and CPR Certification
Uses limited independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issues to supervisor and when to handle them personally.
Knowledge and Skills
Has a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has working knowledge of the organization.
