Responsible for supervising the day-to-day operations of the Skilled community. Adheres to the community standards and maintains the highest degree of quality care at all times. Remains in accordance with federal and state regulations.


  1. Assists the Director of Clinical Services in managing the day-to-day operations of the skilled clinical services department during the scheduled hours.
  2. Assesses the needs and development of residents and implements comprehensive care plans accordingly. Consults with residents’ physician in planning resident care. Periodically reviews and updates comprehensive care plans, ensuring they are interdisciplinary and updated by the Charge Nurse as changes occur.
  3. Works with interdisciplinary team members in developing a discharge plan for individual residents and assists in achieving discharge planning.
  4. Conducts resident rounds to assess and/or supervise nursing care, assess condition of residents, and ensure a safe, clean, and comfortable environment. Assists Charge Nurses with resident care needs and audits documentation. Communicates with and acts as a liaison between residents, family, physicians, and associates.
  5. Participates in developing and maintaining policy and procedures that govern the clinical services day-to-day operations. Makes written and verbal recommendations to the Director of Clinical Services concerning the operations of the nursing department.
  6. Participates in associate evaluations and administers disciplinary action as requested.
  7. Ensures proper transition of nursing reports upon starting and ending shift duty hours.
  8. Completes follow-up investigations on incidents/accidents and provides, or arranges for, follow-up education as indicated. Reports occupational exposure to blood, bodily fluid, infections materials, and hazardous chemicals in accordance with community policy.
  9. Ensures assigned work areas (i.e. nurse station, medication cart, medication room, etc.), resident care rooms, and treatment areas are maintained in a clean and sanitary manner. Monitors supplies and places orders as needed. Ensures medication room and medication cart are locked according to regulations.
  10. Follows up on any pharmacy recommendations to assure response and implementation of order changes.
  11. Attends regularly conducted staff meetings as scheduled by the Director of Clinical Services/Nurse Manager and participates in continuing education in-service programs. Attends various committee meetings as required by the Director, Clinical Services.
  12. Completes other duties as assigned.

Education and Experience
Graduate from Accredited Nursing School. Minimum one to three years’ experience in nursing service administration, including one year of management experience. Rehabilitative or geriatric nursing experience; or equivalent combination of education and/or experience preferred.


Certifications, Licenses, and Other Special Requirements
Current RN state nursing certifications in good standing with no actions on nursing license. Current CPR and First aid certifications required.

Uses independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issues to supervisor and when to handle them personally.


Knowledge and Skills
Has a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has a working knowledge of the organization. Has basic level computer skills including Outlook, Word, and Excel. Effective written and oral communication skills. Ability to respect the privacy of residents and to refrain from discussing their protected health information with other residents/staff, particularly as it may relate to their health. Ability to create and maintain an atmosphere of warmth, personal interest, positive emphasis, and a calm environment.
